New Patient Information

Welcome to High Desert Naturopathic Care. If you would like to schedule an appointment with Dr. Keller, please call 505-670-9042 or send an e-mail to info@naturecure.net. To learn more about Dr. Keller and our clinic services, click here. If you have already scheduled an appointment with Dr. Keller: Patients are asked to arrive about 15 minutes prior to the first scheduled appointment in order to fill out paperwork. If you prefer, you may download the paperwork found at the bottom of this page and fill it out at your leisure. Please bring a completed intake form and financial agreement to your first office call. Other helpful information: Labwork: If you have copies of pertinent labwork, please bring them with you. If you have had recent labwork done, but do not have a copy, you can fill out a release form during your visit. Medications: It is helpful to bring any medications that you are currently taking.
